Applicants: please open, download the documents, and make a copy before you type in them.

If you are accepted, NHS new applicant-members: please click the link to complete the acceptance tasks before midnight on March 26th.
ALL NHS CHAPTER MEMBERS NEED TO HAVE THIS FORM ON FILE WITH THE ADVISOR PRIOR TO PARTICIPATING IN ANY CHAPTER ACTIVITIES.
